Scheduling and Form Uploads

Client Self Complete Online Intake Form
- YES – Client Has Declaration Pages
  - Check-Up: Client is redirected to schedule. HTA receives intake, dec pages, and a reminder to confirm scheduling.
  - Quick Quote: Client is advised quotes will be sent within 3–5 business days. HTA receives intake, dec pages, and a reminder to log the request.
- NO – Client Does Not Have Declaration Pages
  - Client receives email with link to upload declaration pages later.
  - Once uploaded, they can schedule a Check-Up or request Quick Quotes.
  - HTA receives the dec form submission and review request.

HTA Complete – Dec Pages not rec’d yet
- ⁠Sends Hipaa Form Update to AutoHome
- Redirects to INTERNAL PAGE with
  - Reminder to send email requesting DECS
  - Reminder to Schedule LIVE DC if not a prescheduled call
HTA Complete – Dec Pages already rec’d
- ⁠Sends Hipaa Form Update to AutoHome
- Redirects to Schedule Check Up or QQ
  - When Check Up or QQ Scheduled
    - Zaps to EZ Activity and Sales Center
    - Sends appt confirmation to AutoHome
Plan Review Intake and Schedule Form
- Redirects to schedule Plan Review for Existing clients
Intake 2 Check-in and Schedule Form
- Re-direct to Checkup appt for Intake 2 clients
Doc Upload Form
- Re-directs to schedule PL RM appt once dec pages uploaded
General Upload Form
- Secure site to upload docs only.
